| I understand that
participating in a golf tournament is a potentially hazardous activity.
I understand that I should not enter unless I am medically able
and properly trained. I appreciate and assume all risks, both known
and unknown, associated with participation in this event including,
but not limited to: falls, contact with other participants, the
effects of the weather, injury from equipment, etc. In the event
of an accident, injury or illness of the above-named participant,
consent is hereby given to emergency care, emergency transportation,
X-ray examination, anesthetic, medical or surgical diagnosis or
treatment, and hospital care which is deemed advisable by, and is
to be rendered under the general or special supervision of, any
physician licensed under the provision of the Medical Practice Act.
You are responsible for proper conduct and attire during the
event and any associated activities before or after. Each participant
is responsible for the care of the golf course, club house facilities,
equipment and golf carts. The presenters, sponsors and golf course
staff reserve the right to remove any player or team for inappropriate,
disruptive, violate, profane, intoxicated or abusive behavior.
Having read this Waiver and Release Agreement and knowing these
facts, and in consideration of this entry, on behalf of myself,
my heirs, executors and administrators, I hereby waive any and
all claims I may have for damages against Adrenaline Chicks, including
all sponsors and sponsored individuals associated with the event,
as well as each party’s representatives and successors,
and assigns, for any and all injuries and liability suffered by
me in connection with this event, including pre- and post-golf
activities. I hereby grant permission to Adrenaline Chicks and
its authorized agents to use my name and photographs, videotapes,
motion pictures, recording and any other record of my participation
in this event for any purpose.
I have carefully read and clearly understand this Waiver and
Release Agreement. |
